MAJOR DUTIES: 1. The purpose of this job position is to produce diagnostic images utilizing high-frequency ultrasound equipment, and to instruct others in acquiring these skills. Provides complex cardiac sonographic studies (two-, three-, and four-dimensional sector scanning, color, specular Doppler and myocardial mechanics) in adult patients referred to cardiology or through other department consults. This will include, but is not limited to transthoracic, exercise and pharmacologic stress, and trans-esophageal studies. Receives and reviews requests for ultrasound examination from physicians to determine the type of procedure required and the appropriate format of the examination. Assess for potential contraindication for procedure being requested. Performs pharmacologic and exercise stress testing, tracks and logs all of the above studies. Oversees the organization, filing, and retrieval of cardiology patient medical information from clinic, laboratory, and professional correspondence for cardiology staff and fellow physicians. Reports on studies performed regarding critical results and information found during sonographic procedures. Communicates the ultrasound examining process to the patient so that maximum patient cooperation can be obtained. Positions the patient properly so as to record the best possible images. Sets up, adjusts, and maintains the ultrasound equipment so that examinations are of the highest possible quality. Performs detailed ultrasound examinations, modifying the type of examination based on pathology discovered during the procedure. Will utilize different techniques on own initiative so that the examination will provide the most detailed information in assessing the patient's diagnosis. Informs the physician of any aspects of the examination that may have a bearing on the diagnosis or treatment of the patient and uses judgment in requesting physician interaction in the procedure itself. Operates a data entry terminal or computer to input or retrieve data and reports. Maintains and cares for all ultrasound equipment, performing preventive maintenance and identifying machine malfunctions requiring repair. Obtains necessary repairs on such equipment, utilizing the appropriate administrative procedures and interacts with factory representatives. Ensures that adequate supplies are ordered on a continual basis, including imaging materials such as recording paper, videotapes, CDs, DVDs, etc. Establishes and maintains patient records and files related to their examinations and maintains a log of patient visits and types of procedures performed. Reports workload, supply status, and equipment status on a monthly basis. Assists cardiologists in performing invasive diagnostic procedures, such as intravenous contrast injection, trans-esophageal echocardiography, and imaging during ricardiocentesis. Accompanies physicians for performance of special procedures in the treadmill room and in operating room for open-heart diagnostic imaging. Utilizes the echocardiographic scanner to properly position patients for procedures and advises physicians on results of interventions performed during the procedure. Identifies indications and contraindications for the use of echocardiographic contrast and/or agitated saline studies. 2. Coordinates and demonstrates advanced sonography training protocols for physicians, medical students, nurses, Cardiology fellows, Sonographers and Cardiovascular students in a dynamic and demanding patient care environment. Communicates closely with Army Medical Department Center and School professional staff and adheres to strict compliance with approved student education plans. Provides direct feedback to Lab director and senior Cardiology physician staff regarding performance of Cardiology fellows, sonographers and cardiovascular students in applied sonography techniques. Oversees testing of students during hands-on phases of training. Assists with student and physician didactic mentorship to include referencing advanced level literature and ensuring accurate interpretation and implementation of study material. Takes action to correct training deficiencies and supervises follow-up training programs that are deemed necessary. Examines patients in the special care units of the hospital, while continually monitoring their response to the examination and procedure. May render emergency CPR or other aid as required and summons assistance from other medical personnel. Utilizes own judgment in requesting the presence of a physician or nurse during the procedure to assist inn safe patient examinations. Reports findings to the appropriate cardiology staff per the lab's standard operating procedure. 3. Trains physicians and students in ultrasound examination equipment use, procedures, and interpretation of echocardiographic images. Develops ultrasound-training programs for presentation of didactic material. Explains procedures and limitations of methodology, while instructing in the use of ultrasound as a diagnostic tool. Administers or supervises the administration of echocardiography contrast and saline. Attends in-service training to keep abreast of new and innovative sonographic procedures, technology, and techniques. Monitors clinical quality control and hospital safety in accordance with manufacturer, Joint Commission on Accreditation of Healthcare Organizations and Occupational Safety and Health Agency guidelines. In the absence of a noninvasive lab director, oversees the work of all technicians and students, assuring all studies are performed in an expeditious manner. 4. Coordinates laboratory activities to maximize equipment utilization and patient care by sections and departments requiring these services. Performs duties in a timely and quality manor as dictated by the echocardiography lab director. Effectively communicates with referring physicians and departments to maximize patient flow and provider efficiency. Reviews current literature and ICAEL/ASE documents for quality control and current standards as well as reviews and implements advances in technology. Performs other related duties as assigned.
