Outpatient Internal Medicine Nurse Practitioner/Physician Assistant

We are currently seeking a dedicated and experienced Nurse Practitioner or Physician Assistant to join our team in our Outpatient Internal Medicine department. As an Outpatient Internal Medicine Nurse Practitioner/Physician Assistant, you will play a vital role in our patient care team, working collaboratively with physicians, nurses, and other healthcare professionals to provide exceptional medical care to our diverse patient population. Your responsibilities will encompass patient assessments, diagnosis, treatment, and ongoing management of patients with various medical conditions. This position offers the opportunity to work in a supportive and innovative healthcare environment while promoting continuity of care and building lasting patient relationships. This is a full time 40 hour a week position with same day or next day clinical access.

Key Responsibilities:

  1. Patient Assessment and Evaluation: Conduct thorough medical assessments, including medical history, physical examinations, and diagnostic tests, to determine the patient's health status.

  2. Diagnosis and Treatment: Formulate accurate medical diagnoses and develop evidence-based treatment plans in collaboration with the supervising physician.

  3. Medication Management: Prescribe and administer medications, as well as educate patients on medication adherence and potential side effects.

  4. Patient Education: Provide patients and their families with comprehensive education on their medical conditions, treatment options, and preventive measures.

  5. Routine Follow-up: Monitor and manage chronic medical conditions, and schedule routine follow-up appointments to assess treatment progress and make necessary adjustments.

  6. Health Promotion: Promote preventive care, healthy lifestyle choices, and disease prevention strategies to improve overall patient health and well-being.

  7. Collaboration: Collaborate with interdisciplinary healthcare teams to ensure seamless patient care coordination and communication.

  8. Documentation: Maintain accurate and up-to-date medical records, documenting patient visits, treatment plans, and outcomes in compliance with healthcare regulations.

  9. Quality Improvement: Participate in quality improvement initiatives, contribute to clinical guidelines, and stay updated on the latest medical advancements and research.

Qualifications:

  • Current Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Oregon
  • Certification as a Family Nurse Practitioner (FNP) or Physician Assistant-Certified (PA-C).
  • Minimum of 1 yearclinical experience in internal medicine or a related field preferred.
  • Strong clinical assessment and diagnostic skills.
