Physician Employment Agreement:
Malpractice coverage (Tail out of current carrier and convert to new policy) Contract Term three years. First year guaranteed base plus bonus potential, second and Third year convert to production/performance contract terms % of professional and ancillary receipts paid in bonus greater than base compensation year one. Year two: % of receipts $3,500 CME 5 days CME education 15-20 Vacation days, 7 HMA holidays, accrual of 1 sick day per month $400.00 books Pay 5 association dues Reimburse lis, hospital credentialing Stipend for cell phone, lab coats Health, dental, vision benefits.
