Financial:
- Competitive Base Salary ($190K) with productivity incentives
- 2 year contract
- Sign on bonus: $10,000
- Relocation expenses: $7,500

- Dues/Membership/CME fees: $5,000 per year for documented expenses
- Malpractice liability: Covered by practice
- Personal Leave: 4 weeks (15 days) paid time off for sick/vacation time
