Washington Geriatrics

Seattle, Washington -- Providence ElderPlace has an outstanding opportunity for a part time day shift Geriatric Medicine physician. Board Certified in Internal Medicine or Family Medicine required. Generous compensation and full benefits, even for 0.5 FTE. Responsible for direct medical care of PEPS participants, including physical assessment, diagnosis and treatment. As a member of the Multidisciplinary Team, participates in the development of an integrated plan of care to meet the individual's health and social needs. Follows patients across the continuum of care, admitting to inpatient and emergency care settings as appropriate, and serves as liaison to the medical community on behalf of Providence ElderPlace-Seattle. Functions as a member of the Leadership Team and supervises the performance of a Nurse Practitioner. At least one year of experience in medical practice for the chronic care population required. Certificate of added qualification in Geriatrics and/or experience in the care of the chronically ill preferred. Experience working with an interdisciplinary team and performing comprehensive geriatric assessments preferred.
