The Elder Service Plan (of Harbor Health Services Inc.) is a PACE program (Program for All Inclusive Care for the Elderly), an innovative, comprehensive, capitated program designed to maintain frail elders in the community. The program is one of only 42 in the country and offers a unique opportunity to work with a collaborative interdisciplinary team to manage the care of very frail elders. As a physician in the program, which is both the insurer and the provider of care, you would be empowered to design the best plan of care for your patients. The interdisciplinary team provides you with on-site collaboration and support for their care. Due to the frailty of the patients and the need to closely monitor their care, the caseload averages 150-200 patients and is shared with two nurse practitioners.
We have about 270 participants currently in our PACE program. The Physician coming on board would be taking about half of that population, anywhere between 100-150 patients. About 80 % of the population have some degree of dementia and suffer from the usual range of chronic diseases including Coronary Artery Disease, Diabetes, Vascular Disease, and Renal Insufficiency. We provide the full range of care in the Adult Day Health Center including Medical care, Physical Therapy, Occupational therapy, Recreational therapy, social services and counseling for families and patients and psychiatric care if needed.
Salary is $135k to $192K depending on relevant experience and a comprehensive benefits package is offered (brochure attached.) Also, our providers are covered for malpractice insurance under the FTCA (Federal Tort Claims Act.)
