Opportunity Description: Founded over twenty years ago, the center has grown to serve the primary health care needs of its diverse community. Almost all patients are below 200% of federal poverty guidelines for family income. A community board of trustees the majority of whom are patients of the practice- oversees the practice. The center became Federally Qualified Health Center under the Bureau of Primary Health Center Care in 2000. NFP is committed to reducing health disparities and providing the best community-based care for the inner city patient population, and we are an active partner with many other Cleveland organizations to improve the health of the community.

The family physician evaluates and interprets symptoms and signs leading to the diagnosis and management of medical and psychosocial problems in the office and cares for their own patients in the hospital. Community Type and Description: Cleveland Ohio is a very affordable community for housing with many professional sports and excellent cultural activities. Service Area Size: Serving eight neighborhoods, near west side of Cleveland.

Type of Practice: Family Practice Health Center

Practice Environment: Staff of 60 employees, six board certified family practice physicians, two certified nurse midwifes, two nurse practitioners, and five mental and behavioral health providers.

Practice Schedule: 8:30 a.m. to 8:30 p.m. Monday and Tuesday; 8:30 a.m. to 5:00 p.m. Wednesday, Thursday and Friday. Providers scheduled for one evening each week.

Salary: Beginning at $120.000 for full-time, incentive for visit volume.
