Are you seeking an opportunity to provide hands on care and truly make a difference in patients lives?
VNAs HouseCalls program delivers primary medical care, including assessment, coordination of services, diagnosis, health maintenance, and palliative care, through a team of Physicians and APNs to homebound individuals challenged to travel to medical facilities to receive this care.
We seek skilled Physicians with:
1.Ohio MD licensed to practice medicine.
2.Min 5 yrs experience as a physician in providing medical care (internal, geriatric, or family medicine) in community based settings preferably in the home environment. Knowledge regarding home health and hospice care.
3. Knowledge and expertise in the medical management of elderly patient with chronic progressive illnesses; emphasis on pain and symptom control, psycho social management, and disease management.
4.Ability to work with Advance Practice Nurses in a collaborative agreement
