Opportunity Description: We have a busy health center with two sites where, about 13,000 patients are seen in 40,000 visits. We provide Ob/Gyn, pediatrics, adult/family medicine, podiatry, asthma/allergy care and general dentistry. We expect volume to grow in all clinics based on the needs in Lorain County and the fact that LCHD is the only safety-net provider of medical and dental care treating patients of all ages.

This opportunity is for our satellite site at 3745 Grove Avenue where all family practice is located. Family practice sees patients ages eight years and up.

Community Type and Description: Lorain is the largest city in Lorain County and is situated in the northern most point of the county on Lake Erie. Residents enjoy the lakefront, a culturally diverse population, and the citys good and reliable services. Lorain is an easy 40 minute drive to Cleveland and a 30 minute drive to the Cleveland Hopkins International airport.

Lorain offers urban, rural and lake/riverfront living. New and older neighborhoods have single family homes and condo developments. Land is available for new home construction in the city and surrounding areas.

Near Lorain are smaller cities such as Oberlin (with its college, art museum, concerts and community events), Amherst (sandstone capitol of the world), and Vermilion (known for boating, fishing and lakefront living). Sandusky is 40 minutes west of Lorain and is home to Cedar Point Amusement Park (voted the nations best roller coaster park) and the ferry boats to the Lake Erie Islands.

Service Area Size: Service area includes the City of Lorain in Lorain County with approximately 70,000 residents. Located on Lake Erie 30 miles west of Cleveland, Lorain was built on manufacturing and has a very culturally-diverse population.

Type of Practice: Community health center providing primary care.

Practice Environment: Family practice at the satellite in a modern one-story medical building. The space was specifically designed for outpatient use and the environment is comfortable and pleasant. The physician will be assisted by an LPN or medical assistant. There is appropriate support staff at registration, discharge and in medical records. Bilingual staff members are available for translation. The family practice clinic has an active roster of about 4,000 patients annually. The clinic has a 40-year history of providing community health care; the last six years as an independent, federally qualified health center. Many families are second or third generation patients of the health center. Lorain County Health & Dentistry has an excellent reputation in the medical and general communities. Providers, support and enabling staff and administration work closely together to achieve defined goals. Specialists in the community accept LCHD referrals, some only charging income-based fees.

Practice Schedule: Monday-Friday from 8:30 am to 5 pm. Some evening hours may be required.

Salary:Base salary of $115,000. Opportunity for productivity-based compensation after one year (required after two years).

Benefits:Benefits include medical, dental and vision coverage; generous paid time off, life insurance and short and long-term disability; 401k with 100% company match up to 4% after one year, plus CME and membership allowance. Employment Arrangements: Scheduled work week is 40 hours, including about 36 clinical appointment hours/week. No inpatient responsibilities. Two-year employment contract.
