This home visit practice model focuses on improving the quality of life and health care of frail, elderly patients with chronic, complex medical conditions through regular care in the home. The ideal physician would be a Board Certified/Board Eligible Family Practice, Internal Medicine or Geriatrician physician with an active Michigan license. Must be comfortable going to patients' homes and sharing call with team members. Physicians will receive a strong base salary, laptop computer, cell phone, medical supplies, car allowance and a comprehensive benefits package..
