Indiana Family Practice

Under the direction of the Medical Director, provides clinical care consistent with the mission, values, and goals of Hilltop Community Health Center (HCHC). Must maintain appropriate qualifications and provide documentation of current credentials including graduation from approved medical school; residency program completed; board, DEA, and CPR certifications; and Indiana State Medical License. The Physician will be responsible for the following responsibilities: Utilizing appropriate and effective patient care plans; managing patients to assure continuity of care; provide whole-person care; utilizing cost-containment measures; maintaining adequate, concise, and legible documentation of patient care; provides appropriate patient education; focuses on satisfaction of patient and other customers; demonstrates appropriate work habits; and provides hospital care as assigned. The physician will also be expected to contribute to team efforts and participate in networking and community-oriented activities.
