Community Reach Center seeks a full-time BC/BE Primary Care Provider to work with our partners at Mountainland Pediatrics, Inc. The Provider will have responsibility for direct comprehensive medical evaluation, treatment planning and primary care intervention for children and adolescents. DEA license and license to practice in the State of Colorado required.

Duties and Responsibilities:

  • Examines, evaluates and treats medical illness and injuries of infants, children, adolescents and young adults.
  • Determines the need for clinical laboratory tests and other interventions.
  • Interprets examination findings and test results, and implements treatment plans.
  • Monitors physical, psychosocial and developmental growth while conducting age appropriate screening.
  • Prepares and reviews cases to staff with appropriate supervisor and/or collaborating physician.
  • Provides advice and education for consumers and parents regarding appropriate preparation for predictable developmental challenges.
  • Determines need for consultation and assists in medical care treatment provided by the direction of other specialists.
  • Participates in program planning of the integration of primary care and mental health.
  • May instruct medical students, residents and other trainees in procedures for diagnosis and treatment of medical illnesses and injuries.

Formerly known as Adams County Mental Health Center, Community Reach Center is now one of the premier providers of counseling and mental health services in the Denver area. We service residents in Adams County and the north metro area by providing evaluations, counseling, and therapies to a wide range of consumers, including adults, adolescents, children, families, and seniors. Our staff is committed to understanding and improving the mental and emotional health of the people we serve by providing caring, confidential, high-quality and cost-effective services.

    We partnered with Mountainland Pediatrics, Inc. in January of 2008, with the goal that over time, mental and physical health needs of our consumers could be met all at once. Community Reach Center is one of only a few community mental health centers that have taken a step towards this goal.
