Shareholder status after 2 years, financial package includes first year salary $320 - $360K, with $10K sign-on bonus, production bonuses, malpractice, health includes family, LT/ST disability, life, dental, 6 weeks vacation, 1 week CME w/$4K expenses, pension, relocation, buy-in for $500.
