Post Acute Physician - Team Lead

with $25k Signing Bonus

Sound Physicians is committed to improve quality outcomes, reduce avoidable readmissions and enhance the patient experience. This position places you as a team leader with the autonomy to improve care transitions for patients who are discharged from one of our Flagship Hospitalist programs.

You will have complete logistical support. Our post-acute support program provides portable technology, mileage reimbursement and flexible scheduling. There is an opportunity to be Medical Director of facilities and receive an additional stipend.

•You will lead a team of two highly-skilled, independent Nurse Practitioners

•The schedule is flexible, approx. 8 hour days, Monday-Friday

•As the Team Lead, you will be geographically based, covering only three high-functioning post-acute facilities around Bremerton, WA. We already have excellent relationships in place.

•You will have minimal call as the ARNPs are primary call & you would serve only as backup. Currently, there are less than 5 phone calls per month

•Responsible for admissions and for executing the post discharge plan of care

•Responsible for ensuring safe discharge to home from the Skilled Nursing Facilities

•Oversees Nurse Practitioners Providing Transitional Care services in three Skilled Nursing Facilities

Minimum Qualifications

•MD or DO with active and unrestricted license in WA

•Minimum of 3+ Years of experience working with Geriatric population - Preferred

•Board Certified or Board Eligible in Internal Medicine, Family Practice or Geriatric Medicine

•Excellent communication (written and verbal), analytical and problem-solving skills
