Full time Geriatric Home Health with some virtual in Phoenix! JOB ID 71981

Job Details:

BC in FM, IM or Geriatric (or have an interest in Geriatrics)
At least two years of clinical experience with home-based care and/or Geriatric Medicine is strongly preferred
Primary Care for Seniors majority home-bound
6-8 patients per day ( For follow up visits, Physician decides if it will be virtual or in person depending on the patient)
Patients' homes within driving distance of the Physician
They have a mobile lab and xray when patients can't go to a nearby facility
Provide laptops or tablets EMR is Athena
Reimburse the federal rate for mileage
Care Team Model
80% patient care (mostly home visits, some virtual) and 20% admin (done from home)
On call nights and weekends by phone 1 week per month, as well as backup weeks
