This role involves collaborating with healthcare teams, assessing patient requirements, diagnosing conditions, and devising treatment strategies.Responsibilities:- Plans and organizes patient care according to protocols and pathways.- Collaborates with the healthcare team to develop and implement treatment plans.- Conducts health assessments and orders necessary tests.- Prescribes medications and medical equipment as per practice protocols.- Evaluate treatment effectiveness and modify plans as needed.- Promotes professional growth and serves as a clinical resource.- Assists Clinical Access Center staff when necessary.- Completes additional tasks as directed by the supervisorQualifications:- Current registration as a Nurse Practitioner in New York State- Master's degree in nursing- Certification as an Adult or Family Nurse Practitioner
