Amiodarone pulmonary toxicity after lung transplantation.
Diaz-Guzman E, Mireles-Cabodevila E, Arrossi A, Kanne JP, Budev M Related Articles Amiodarone pulmonary toxicity after lung transplantation. J Heart Lung Transplant. 2008 Sep;27(9):1059-63 Authors: Diaz-Guzman E, Mireles-Cabodevila E, Arrossi A, Kanne JP, Budev M Atrial fibrillation occurs frequently after lung transplantation and is commonly treated with amiodarone. Pulmonary toxicity may result from amiodarone exposure and is characterized by non-specific respiratory manifestations. To our knowledge, there are no reports of this complication occurring after lung transplantation. We present a patient who developed radiologic evidence of amiodarone deposition in the lungs after bilateral lung transplantation. PMID: 18765204 [PubMed - in process]
Late unilateral pulmonary edema in single lung transplant recipients.
Akindipe O, Fernandez-Bussy S, Staples ED, Baz M Related Articles Late unilateral pulmonary edema in single lung transplant recipients. J Heart Lung Transplant. 2008 Sep;27(9):1055-8 Authors: Akindipe O, Fernandez-Bussy S, Staples ED, Baz M In this study we describe 3 single lung transplant recipients who developed unilateral pulmonary edema in the setting of cardiac and renal dysfunction. All 3 patients responded to diuresis with clinical and radiographic improvement. Unilateral cardiogenic pulmonary edema should be considered in the differential diagnosis of dyspnea and unilateral radiographic infiltrates in single lung transplant recipients. PMID: 18765203 [PubMed - in process]
Late allograft failure after heart transplantation: 2 unusual cases.
Boilson BA, Miller DV, Pereira NL Related Articles Late allograft failure after heart transplantation: 2 unusual cases. J Heart Lung Transplant. 2008 Sep;27(9):1050-4 Authors: Boilson BA, Miller DV, Pereira NL Cardiac allograft failure is most commonly caused by acute rejection and cardiac allograft vasculopathy. Consequently, heart transplant recipients who present with heart failure are empirically treated at many transplant centers with augmented immunosuppression despite negative endomyocardial biopsies. We report 2 unusual cases of acute allograft dysfunction that highlight the importance of caution in adopting an empiric strategy of profound immunosuppression in the absence of objective data to support graft rejection. PMID: 18765202 [PubMed - in process]
Native lung-sparing lobar transplantation for pulmonary emphysema.
Yamane M, Okutani D, Sugimoto S, Toyooka S, Aoe M, Okazaki M, Sano Y, Date H Related Articles Native lung-sparing lobar transplantation for pulmonary emphysema. J Heart Lung Transplant. 2008 Sep;27(9):1046-9 Authors: Yamane M, Okutani D, Sugimoto S, Toyooka S, Aoe M, Okazaki M, Sano Y, Date H The living-donor lobar lung transplantation procedure has been developed clinically as an alternative approach for patients considered too ill to await cadaveric transplantation. With this procedure, 2 lobes are implanted in the recipient in place of whole right and left lungs, respectively. However, the shortage of graft volume can be a problem when compared with full-sized cadaveric grafts. In an attempt to solve this problem, we have developed a native lobe-preserving lobar transplant technique using a large animal model. We report a first successful case of a patient undergoing native lobe-preserving lobar lung transplantation for severe pulmonary emphysema. PMID: 18765201 [PubMed - in process]
Prolonged basiliximab use as an alternative to calcineurin inhibition to allow renal recovery late after heart transplantation.
Anselm A, Cantarovich M, Davies R, Grenon J, Haddad H Related Articles Prolonged basiliximab use as an alternative to calcineurin inhibition to allow renal recovery late after heart transplantation. J Heart Lung Transplant. 2008 Sep;27(9):1043-5 Authors: Anselm A, Cantarovich M, Davies R, Grenon J, Haddad H Solid-organ transplant patients can have a number of complications, some of which are particularly severe with regard to morbidity and mortality. These complications may be due to operative and post-operative complications or to the immunosuppressive therapy regimen. Renal impairment is a frequent post-transplant complication that may occur for any number of reasons, including peri-operative ischemic events and nephrotoxicity secondary to anti-rejection medications. Calcineurin inhibitors (CNIs) in particular can lead to acute renal failure (ARF). There are no clear guidelines for the management of ARF after heart transplantation. Different strategies include delaying initiation of CNI therapy post transplant, and the use of anti-CD25 monoclonal antibodies. PMID: 18765200 [PubMed - in process]
Non-heart-beating lung donation: how big is the pool?
Nijkamp DM, van der Bij W, Verschuuren EA, Heemskerk MB, de Buijzer E, Erasmus ME Related Articles Non-heart-beating lung donation: how big is the pool? J Heart Lung Transplant. 2008 Sep;27(9):1040-2 Authors: Nijkamp DM, van der Bij W, Verschuuren EA, Heemskerk MB, de Buijzer E, Erasmus ME Lungs from non-heart-beating (NHB) donors are seldom used in The Netherlands despite the good quality of these organs. Based on a retrospective analysis of 162 NHB donor procedures we found that only 5% of the lungs were actually utilized, but that 30% of the lungs were suitable for transplantation. Not recognizing the suitability of NHB lungs is likely the main reason for their non-availability. PMID: 18765199 [PubMed - in process]
