Thanatophoric dysplasia: Role of 3-dimensional sonography

Fetal thanatophoric dysplasia (TD) is a lethal skeletal dysplasia. Therefore, antenatal diagnosis of TD is mandatory in routine obstetrical care. However, because TD is relatively rare, prenatal detection is not an easy task. In the past, 2-dimensional (2D) sonography had been applied as the mainstay of prenatal diagnosis of TD. In this series, we report our work of detecting TD using 3-dimensional (3D) sonography.We reviewed our computer database of prenatal diagnosis of TD in National Cheng Kung University Hospital from May 1995 to June 2006. All the cases were scanned using 2D and 3D sonography. In total, 9 cases of fetal TD were diagnosed.3D sonography can detect fetal TD and provide additional vivid illustration after various modes of reconstruction that 2D sonography cannot afford.3D sonography may contribute significantly to the detection of TD in utero and provide a novel visual depiction of this defect after reconstruction. Thus, 3D sonography may assist substantially in prenatal diagnosis as well as consultation. © 2008 Wiley Periodicals, Inc. J Clin Ultrasound, 2008
Endothelial function in patients with and without diabetes mellitus with different degrees of coronary artery stenosis

To evaluate endothelial function using flow-mediated dilatation (FMD) of the brachial artery in patients with and without diabetes mellitus (DM) with different degrees of coronary artery stenosis.We investigated 293 patients, 69 (23.6%) of whom had DM. FMD and coronary arteriography were performed.Patients with DM had a significantly lower FMD (mean ± SD, 3.7 ± 3.8%) compared with patients without DM (mean ± SD, 5.2 ± 5.3%) (p < 0.05). When the results were broken down by the severity of coronary artery disease (CAD) (no significant coronary artery stenosis, 1-vessel disease, 2-vessel disease, and 3-vessel disease) the only statistically significant difference between diabetics and nondiabetics was found in patients without significant coronary stenosis (mean FMD ± SD: 5.2 ± 4.4% in diabetics, 7.6 ± 5.4% in nondiabetics [p < 0.05]).In CAD patients, the presence of DM was associated with endothelial dysfunction. The difference in the FMD was clearly expressed between patients with and without DM in the subgroup without significant coronary stenosis, and was no longer present with advanced coronary atherosclerosis. © 2008 Wiley Periodicals, Inc. Sonographic study of the development of fetal corpus callosum in a Chinese population

The observation of fetal corpus callosum (CC) is important for the prenatal sonographic assessment of fetal central nervous system development. The aim of this study was to investigate the development of normal Chinese fetal CC.CC measurements were performed using high-resolution transabdominal sonography on 622 Chinese fetuses between 16 and 39 weeks' gestation. The correlation between CC size and gestational age was investigated.The fetal CC length increased in a linear fashion during pregnancy. The length of the CC as a function of gestational age was expressed by the following regression equation: length (mm) = -9.567 + 1.495 × gestational age (weeks) (r = 0.932, p < 0.001).Knowledge of normal CC appearance may help identify developmental anomalies and enable accurate prenatal counseling. © 2008 Wiley Periodicals, Inc.
